Resolved survival entry/exit column indices for a saved survival model.
entry_col is None when the model was trained with the right-censored
shorthand Surv(time, event); callers synthesize a zero entry time per
row in that case via SurvivalTimeColumns::row_entry_time. Mirrors
the CLI predict path so every site that consumes saved survival
metadata applies the same fallback contract.
