Metrics collector for the server.

All atomic counters are bare AtomicU64; the collector itself is wrapped in Arc<MetricsCollector> at every use site (see AppState::metrics). The previous per-field Arc<AtomicU64> was a redundant second indirection that bloated the struct to ~30 cache lines and made construction 30 atomic refcount bumps (F009). Call sites are unchanged — metrics.foo.fetch_add(...) resolves to a single Arc deref + atomic op.
