A Vec of Indexes with the given offset type

Tracks other than the lead-out are required to have at least one INDEX 01 index point, which specifies the beginning of the track. An INDEX 00 pre-gap point is optional.

MAX is the maximum number of index points this can hold, including the first. This is 100 for CD-DA (00 to 99, inclusive) and 254 for non-CD-DA cuesheets.
