Allocation-free hot-path result of one structured-output mask operation.

§phase: StructuredOutputPhase§accepting: bool§liveness_intervention: bool§grammar_start_token_index: Option<usize>

Generated-token index at which the visible grammar-owned output starts. None means the processor is still in the hidden pre-grammar domain. The execution engine uses this boundary to scope request-local sampling history without teaching the grammar about penalty policy.

§required_delimiter_token_id: Option<u32>

Exact delimiter token authorized for the next sampling step while the processor is waiting to activate. The engine uses this typed grant to avoid rejecting an intentionally hidden special token during output quality filtering.
