Whether ingest is being starved of sanitize permits, for /stats and the logs (review of #274).

Why: one feed holds at most one thread (InFlight), but four hostile feeds — four URL variants of one, say — can still hold all SANITIZE_CONCURRENCY permits with sanitizes nobody can cancel. Every other feed’s poll then defers (SanitizeGaveUp::NoPermit): nothing is stored and, by design, nothing is filed against those feeds — so without this the instance would silently stop updating. Counted, not prevented: the starvation lasts until those sanitizes finish.
