Host-resolved facts the engine needs but cannot fetch itself.

The host populates only the field relevant to the completed activity, doing I/O in exactly the same conditions as the original planner: act_scheduling only when reason_schedules_act is true, and setup_connection_hint_enabled only when the act paused for tool results.
