Expand description

Adapts a Capability’s Capability::narrate into a ToolCallHook so capability-owned narration flows through the same hook channel the act atom already consults. One is registered per applied capability during assemble, after every explicit tool-call hook, so model-authored narration (e.g. human_intent) still takes precedence.
