A deterministic join failed after some siblings had already completed.

cause retains the typed activity, child-workflow, cancellation, or codec error. completed is declaration ordered and contains only durable successes observed in the same replay. Late sibling completions can add entries on a later replay without changing member_path or the selected positional failure.
