did not apply. Reporting raw accepted=1 conflates these cases; user-facing
acceptance metrics should use accepted_and_applied.
The COALESCE(applied_ok, 1) = 1 predicate treats NULL applied_ok
(older rows from before the column landed on the schema) as success.
