A tool result whose body the host persisted before submitting.

Failure and size are orthogonal (§7.10 rule 9). A tool that fails after producing a huge diagnostic body is not a rare shape — it is the common one — so this arm carries the same two failure facts as ToolResult: whether the call errored, and whether the executor could keep going. Without them an externalised failure was indistinguishable from an externalised success, and a fatal one could not exist at all.
