Struct ErrorContext

Supplies extra context appended to selected tool error messages.

crate::Server places it in each request’s extensions. The callback sees each tool error; when it returns Some(context), the message becomes <message> (<context>) before ToolHandler::redact_error runs, so the context is redacted too.
