# Guardrail & policy

Vallum evaluates every command *before it executes* and returns one of three
verdicts — on all call sites and all agents, including hook mode where
TUI-headed commands like `less`/`vim` are simply never rewritten (see
[SECURITY.md](../SECURITY.md#dangerous-command-execution-guardrail)):
- **Allow** — runs normally (the default for anything no rule matches).
- **Ask** — pauses for explicit confirmation before running.
- **Deny** — refuses to run the command.
The guardrail is **on by default**, and every built-in rule is `Ask` — a
genuinely dangerous command prompts for confirmation instead of running
silently, but nothing is silently blocked. The built-in patterns are
deliberately narrow so ordinary commands are never touched (a committed
benign-command precision gate guards against nagging on legitimate commands).
## How matching works
Matching reinterprets each command through a bounded set of precision-safe
views, so a dangerous command hidden inside a wrapper is still caught: shell
`-c` and `eval` arguments (verb-aware, bare or bundled like `-xc`, behind
`sudo`/`env`/`timeout` prefixes, and nested), `base64 -d` payloads (decoded and
re-checked), `$IFS` splitting, and quote/escape word-splitting applied to both
the payload and the interpreter verb (`\bash`, `b''ash`). Newlines are treated
as command separators. See [SECURITY.md](../SECURITY.md) for the residual known

## Built-in rules
The 28 built-in rules (all default to `Ask`):

Metadata-only commands are exempt: `ls`, `stat`, `file`, `du`, `df`, `chmod`, `chown`, `touch`, `mkdir`, `ssh`, `sftp`, `ssh-add`, `ssh-keygen` |

The six persistence rules gate **writes** only — *reading* a startup file
(`source ~/.zshrc`, `cat ~/.ssh/config`, `git config user.name`,
`crontab -l`) stays `Allow`. The two `vallum`/`~/.vallum` rules close the gap
where an agent turns the guardrail off before doing something dangerous.
## File-tool gating (Claude Code)
The rules above match **shell commands**. An agent can also touch sensitive
files without ever running a command — by calling its editor's native
`Write`/`Edit`/`Read` tool directly. On **Claude Code**, the `PreToolUse` hook
now gates those native file tools (`Write`, `Edit`, `MultiEdit`,
`NotebookEdit`, `Read`) against a set of path rules, so writing `~/.zshrc` or
reading `~/.ssh/id_rsa` through a file tool is caught the same as doing it in
the shell. This is **Claude Code only** in this first version — the other
agents' native file tools are not gated (their shell guardrail is unchanged).
The nine file rules (all `Ask`, like the shell built-ins):

Path matching is **lexical**: `~`/`$HOME` are expanded and `.`/`..` are
resolved textually, but the filesystem is never touched — symlinks are **not**
resolved (see [SECURITY.md](../SECURITY.md)). An `Ask` here carries no command
rewrite (the tool call runs unchanged on approval); the circuit breaker counts
and can deny file-tool calls too; and each verdict lands in `policy.log` as a
`Write ~/.zshenv`-style entry.
These rules share the `[policy] disabled` suppression list with the shell
built-ins (e.g. `disabled = ["file_read_sensitive"]`). Because they ride the
Claude Code hook entry's tool matcher, an **existing install must re-run
`vallum install-hook`** to widen the matcher from `Bash` to the file tools;
`vallum install-hook` migrates the old entry in place, and `vallum doctor`
warns when the matcher is outdated.
## Circuit breaker (blast radius)
When the guardrail returns 5 Ask/Deny verdicts within 60 seconds (a runaway
agent probing dangerous commands), Vallum trips a circuit breaker: **every**
command is denied for 5 minutes — or until you run `vallum unlock`. The trip
itself is recorded in the hash-chained `policy.log` (requires the default
`[audit] sanitized_enabled = true`; the breaker itself works either way). Tune
or disable it under `[security]`:

## How `Ask` surfaces
- **Hook mode (Claude Code, Cursor):** the verdict maps to the agent's native
approval UI — `Ask` prompts you in the agent, `Deny` blocks the tool call.
- **Hook mode (Gemini CLI, Codex CLI):** neither exposes a native "ask"
decision, so `Ask` is enforced as a **Deny** with an actionable reason —
see [Agent integrations](agents.md).
- **Direct `vallum run`:** `Ask` prompts on `/dev/tty`; when there is no
terminal (piped, CI, `--json`) it **fails closed** (blocked) unless you
set `security.assume_yes = true` or `VALLUM_ASSUME_YES=1`. A `Deny`
verdict exits **`125`**.
## Configuring the guardrail
Configure it under `[security]` and `[policy]`:

Setting `guardrail = false` makes Vallum behave byte-for-byte as it did before
this layer existed. User rules are matched with the same most-severe-wins
resolution as the built-ins (`Deny` > `Ask` > `Allow`).
If the config file is broken (TOML or regex error), hook mode logs a warning
to stderr and keeps gating with the **built-in** rules; your custom rules are
ignored until the config is fixed (`vallum doctor` pinpoints the error).
Direct `vallum run` refuses to run at all (exit `125`) on a broken config.
## Scoped allow exceptions
`[[policy.allow]]` suppresses exactly ONE named built-in for commands matching
a pattern — the narrow alternative to disabling the rule entirely:

Rules: `suppresses` must name one of the 28 shell built-ins; the pattern must
compile and must not match the empty string (`.*`-class patterns are rejected
at config load); the exception applies to the raw command line only — an
obfuscated command that fires via de-obfuscation gets no suppression; all
other rules still evaluate. Suppressions that change the outcome are logged to
`policy.log` as `ALLOW [allow_exception:<rule>]`.
## Approval cache
When you approve an Ask (Claude Code hook, or a direct `vallum run` TTY
prompt), Vallum remembers that exact command + working directory for
`approval_cache_ttl_days` (default 14) and stops re-asking. Hard limits, not
configurable: only `git_push_force`, `git_clean_force`, `write_crontab`, and
`write_git_hooks` are ever remembered — destructive commands, credential
reads, agent-config writes, Vallum self-protection, and `curl | sh`-class
rules always re-ask. Entries are HMAC-signed with the machine approval secret;
any tampering, expiry, or mismatch is a cache miss that re-asks. Inspect with
`vallum approvals list`, wipe with `vallum approvals clear`, disable with
`[security] approval_cache = false`. Cache hits are logged as
`ALLOW [approval_cache:<rule>]`.
## Project-level rules (.vallum.toml)
A repo can commit additional `ask`/`deny` rules in `.vallum.toml` at the git
root — and only that; every other key is rejected and the file ignored with a
warning. Discovery: the nearest ancestor with `.git` (worktree `.git` files
included); without a git repo, the current directory. A symlinked
`.vallum.toml` is refused unread — a repo file must not redirect reads outside
the repo. Rules compile as
`project:<pattern>` and show up in `policy test`, audit lines, and
`vallum doctor` (`project-config` check). Scaffold: `vallum config init
--project`. Kill switch: `VALLUM_NO_PROJECT_CONFIG=1`.
## Testing a rule
Test a rule without running the command:

Exit codes: 0 allow, 10 ask, 20 deny, 125 config error — usable from CI.
## Tamper-evident audit log
Every enforced non-Allow verdict is recorded (redacted) to `policy.log`. Every
`policy.log` entry is hash-chained (SHA-256) to the previous one;
`vallum log verify` detects in-place edits, deletions, and insertions, and an
externally stored head hash (`--expect-head`) also catches tail truncation.

## Scope, honestly
The guardrail matches patterns against the command text — it is a speed bump /
defense-in-depth layer, not a sandbox. It sees through common wrappers (see
above), but a determined actor can still get around text matching with
variable or command-substitution indirection. The output-side protections —
secret scrubbing and injection defusal — do not depend on the guardrail and
remain the backstop.
