Echo of EngineOpts.no_extensions — callers gate extension discovery on this so the flag has one source of truth.

Keybind registry. Uses std::sync::RwLock (not tokio) because keybind lookups are synchronous, fast, and called from input handling code that cannot await. This is safe as long as the lock is never held across an await point.
