Rule MD070: Nested code fence collision detection

Detects when a fenced code block contains fence markers that would cause premature closure. Suggests using longer fences to avoid this issue.

Only checks markdown-related blocks (empty language, “markdown”, “md”) since other languages don’t use fence syntax.
