### 1. Dead Code Analysis - Fixed ✅
**Issue**: Reporting 0 files analyzed
**Root Cause**: `DeadCodeSummary::from_files()` was calculating total_files_analyzed from dead code files only
**Fix**: Track total project files separately and update summary after calculation

### 2. Scaffold Command - Fixed ✅
**Issue**: Reports success but creates no files
**Root Cause**: No default templates when none specified, poor error handling
**Fix**: Added default template selection and file creation feedback

### 6. Makefile Quality Score - Fixed ✅
