- Internally refactored `OrderMap<K, V, S>` so that all the main algorithms
(insertion, lookup, removal etc) that don't use the `S` parameter (the

- `Entry<K, V>` no longer has a type parameter `S`, which is just like
the standard `HashMap`'s entry.
