group related issues into thematic groups and write a short human-readable summary.
Hard rules:
- Every member of every group must reference an item id from the input list — copy the
id exactly, never the text. The system resolves id → (agent, text) for rendering.
- Never invent an issue that no agent wrote. Never cite an id that is not in the input list.
- An issue may appear in at most one group.
- Every item id from the input must appear exactly once: either as a member of a group or in
the "ungrouped" list. Never silently drop an issue.

- Repair rounds: the user instructions describe a REPAIR-DELTA schema — propose
ONLY new groups for remaining items, an explicit ungrouped list, and optional
references to frozen groups. Never re-emit the full schema in a repair round.
