Offline builds

The SDK uses sqlx compile-time-checked queries. The published crate bundles a .sqlx query cache so it compiles without a database: when DATABASE_URL is unset, sqlx uses the bundled cache. Set SQLX_OFFLINE=true to force offline compilation even when DATABASE_URL is present.
