## Analysis

### What Worked

✅ **Reduced infrastructure failures** - From 2 to 1 (50% reduction)  
✅ **Improved throughput** - 6.3% faster at 2/2 concurrency  
✅ **Lower latency** - 5.9% faster average latency  
✅ **Fewer lock warnings** - Noticeably less "Database locked" messages in logs

### Why Not Bigger Gains?

The improvement is modest (~6%) rather than the expected 2-3x because:

1. **Lock contention remains the bottleneck** - Still seeing database lock errors
2. **Concurrency level** - Only 2 orchestrator workers competing for locks
3. **Test duration** - 30 seconds may not show full benefit of larger cache
4. **Docker overhead** - Container I/O adds baseline overhead

- Heavy `vfs_write`, `backing_file_write_iter` 
- ~60-70% time in file I/O operations

**Optimized:** `docker_optimized_20251111_103331.svg` (149KB)
- Should show reduced I/O operations
- Better cache utilization

## Key Insights from Flamegraph

### Primary Bottleneck: File I/O (60-70% of CPU time)

**Functions consuming most time:**
- `vfs_write` - Virtual file system writes
- `backing_file_write_iter` - Kernel write operations
- `pwrite64` - POSIX write syscall
- `sqlite_file_write_iter` - SQLite file operations
- `generic_perform_write` - Generic write path

**Root cause:** SQLite synchronous writes to disk

### Secondary Bottleneck: Syscall Overhead (~12%)

**Functions:**
- `invoke_syscall` - 11.76% of samples
- `futex_wake` / `futex_wait` - Thread synchronization
- `Mutex::lock` - Lock operations

### Tertiary: Lock Contention

**Evidence:**
- "Database locked" warnings throughout logs
- Multiple retry attempts with exponential backoff
- Occasional infrastructure failures after max retries

## Recommendations

### Immediate (Already Applied)

✅ Changed `synchronous = NORMAL` → `WAL`  
✅ Increased `wal_autocheckpoint` to 10000  
✅ Increased `cache_size` to 64MB

### Next Steps for Further Optimization

#### 1. Address Lock Contention (Highest Priority)

The flamegraph and logs show lock contention is now the limiting factor.

**Options:**
- **Increase connection pool** from 5 to 10-20 connections
- **Implement connection-level write locks** to reduce contention
- **Batch operations** - Combine multiple work items into single transactions
- **Use `BEGIN IMMEDIATE`** to acquire write lock upfront

#### 2. Reduce Transaction Frequency

**Current:** Each `ack_orchestration_item` is a separate transaction

**Optimization:** Batch multiple acks into one transaction
- Could improve throughput by 2-3x
- Reduces fsync calls
- Better utilizes WAL mode

#### 3. Optimize Serialization (If It Shows Up)

Once I/O is fully optimized, check flamegraph for:
- `serde_json::to_string` / `from_str`
- Consider binary formats (bincode) for internal storage
- Cache deserialized objects

#### 4. Consider Alternative Approaches

**For high-concurrency scenarios:**
- Use PostgreSQL instead of SQLite (better concurrent write handling)
- Implement write-ahead buffer (batch writes in memory)
- Use separate databases per orchestration instance (sharding)
