Generate a concise, commit-style title for the session's durable overall goal.

Rules:

- Use one line in present simple tense, under 72 characters.
- Describe the session's overall requested work, not merely its latest message, the
  assistant's answer, an observation, or an evaluation.
- Stay high-level and intent-focused; omit long file names, paths, and symbol names.
- Treat the original request as the primary anchor. Use the session summary and latest
  request only to clarify or establish the goal when the original request is
  context-only.
- Do not let a narrow follow-up, clarification answer, acknowledgement, or progress
  update replace a broader established goal.
- Omit your progress, checks, reasoning, next steps, and first-person phrasing such as
  "I have", "I'm", or "I'll".
- Do not use Conventional Commit prefixes like `feat:` or `fix:`.
- If the supplied context has no actionable session goal—for example, it is
  conversation, context-only text, or an acknowledgement—leave `answer` empty so a later
  substantive request can supply the title.
- Put only unquoted title text in `answer`, without Markdown fences, explanations, or
  extra text. Leave `questions` empty and set `summary` to null.

Examples:

- Good: `Refactor session lifecycle updates`
- Bad: `I updated the session lifecycle and ran tests`
