How a grafted item’s content is carried into the working set.

The graft is by COW reference — the origin chain entry is never removed. Small text rides inline; large text is referenced by its content-addressed blob hash (load via ArtifactStore); a chunk whose text was not retained in this view is a bare reference to be recovered from the chain.
