Context for ProtocolPolicy::hidden_tool_error.

Describes a tool call the model made for a tool that wasn’t in the turn’s advertised allowlist. The policy renders a model-recoverable error. Only consulted when no crate::plugin::ToolGate attributed the denial via crate::plugin::ToolGate::denial_reason.
