A ScopeConstraint was built with no filters.

A constraint is a conjunction, so an empty one is an AND over nothing: it matches every row. As one disjunct of an AccessScope that makes the whole scope allow-all, while is_unconstrained() and is_deny_all() both still answer false — so the scope looks constrained to every caller that asks. toolkit-db compiled exactly this shape to an unconditional WHERE true.
