Outcome-aware throttle segment (ADR-0025).

Wraps a ThrottlerConfig + shared RateLimiter + child sub-pipeline body. Unlike ThrottlerService (which operates at the Tower layer), ThrottleSegment correctly propagates PipelineOutcome::Stopped / Failed from the body.
