Pin key = (error_signature, operation) per Q6 of the 2026-04-20 spec amendment.

error_signature is <error_type>:<short_hash_of_structural_props>. Structural properties — not exact values — drive the hash so ConnectionTimeout{after: 30s} and ConnectionTimeout{after: 45s} share a signature, while ConnectionTimeout and ConnectionRefused pin independently.
