Identity of one source file inside a ResolvedProgram.

Opaque on purpose. It means nothing outside the program that minted it, and the only way to spend it is ResolvedProgram::file — a caller that could compute a file index is a caller that can compute the wrong one, which is the failure this whole type exists to make impossible.
