Compute the table with read when the first place is asked for.

pub fn locate(&self, key: &str, span: Span) -> Option<Place>

Where span is, inside the definition bound to key. None for a synthetic span, a key with no source, or a span past the end of the key’s file: an offset outside the file is proof it indexes another one, and answering it anyway would name a line that is not the place.
