InvalidGrant— the provider answered 400invalid_grant: the refresh token is dead; the caller expires the account (the user must reconnect), never retries.Provider— the provider refused or returned an unusable shape. Zero writes; the next tick retries.Network— transport-level failure. Zero writes; the next tick retries.EndpointGuard— the guard refused (revalidation or resolution). Zero writes, zero HTTP calls.
