New request stream rejected because peer-advertised concurrency cap is full.

Per RFC 9114 §5.1.2, an HTTP/3 endpoint MUST respect the QUIC initial_max_streams_bidi / MAX_STREAMS limits. The local state machine returns this error when a frame arrives for a previously-unseen request-stream id while active_request_stream_count >= max.
