Fail-closed: Deny action on error (default, safer)
Use when:
- Compliance requirements mandate fail-safe behavior
- Safety-critical environment
- False negatives are worse than false positives
Allow
Fail-open: Permit action on error
Use when:
- Availability is more important than enforcement
- Development/testing environment
- Other layers of defense exist
