Invalid patch envelopes return an ApplyDiffError. Failures involving an individual file are collected here so unrelated files can still commit.

applied: Vec<AppliedFile>

Filesystem changes that committed successfully.

failed: Vec<FailedFile>

Logical files that remained unchanged because planning or commit failed.
