What one Reader::refresh added to a snapshot.

The counts describe only the frames discovered by that call, never the frames the snapshot already held, so the sum of a report’s blocks_added across refreshes is the total number of blocks the refresh path has contributed.

Whether the refresh left a physically incomplete frame after the last complete committed frame.

True here does not mean the snapshot is damaged: an interrupted append exposes no block and a later refresh may see that tail become a complete committed frame.
