Rich hook execution outcome used by governance-aware callers.

HookResult remains the compatibility surface for existing executors. This outcome additionally preserves the explanation attached to a retry so callers can distinguish a temporary denial from a permanent block.

Variants (Non-exhaustive)

Non-exhaustive enums could have additional variants added in future. Therefore, when matching against variants of non-exhaustive enums, an extra wildcard arm must be added to account for any future variants.
