Extract filesystem paths affected by a mutating tool call.

Public apply_patch payloads are decoded and parsed through the same patch grammar used for execution. Generic path-bearing mutation tools retain the established singular and batch argument conventions. Tools whose payloads do not expose paths return an empty vector and cannot invalidate a scoped replay.
