Did this failure happen BEFORE a single byte of the request went out?

A connection that was refused, a name that did not resolve, a connector that gave up: nothing reached the far end, so asking again cannot duplicate anything. A reset or a timeout mid-flight is NOT in this set — the request may well have arrived and been acted on.
