Response composition: what the assistant is allowed to say (spec §10, §17.3, §18, §23 steps Q to U).
By the time this runs everything that could happen has happened, and the job is to
say so accurately. Receipts, notices and cards are rendered by the server from what
committed; the words around them are written by the narration tasks from facts code
selected, and claim_guard::verify checks the assembled
turn against the record before it is returned. Every AnswerTask produces exactly
one block, answered or explicitly not.
