The [mcp_servers.<id>] table Codex reads, or a refusal for a
declaration its config cannot carry.
Codex forwards a variable from the user’s environment under its own
name (env_vars), so a declaration that renames one is refused rather
than written as a literal ${VAR} Codex would not expand. A header is
either a bare variable (env_http_headers) or Authorization: Bearer
(bearer_token_env_var); any other format is refused.
