Plans a bulk enable or disable. names of None means every recorded mod.

A name in names that is not recorded at all is refused outright, in the same shape as plan_uninstall_batch’s refusal, rather than silently producing an empty plan: iterating the record and filtering would let an unrecorded name simply never appear, indistinguishable from “nothing to do”.
