DiscoveryState

Process-level discovery state (daemon-mode phase 2, C2). Discovery walks the plugin roots and spawns sidecars ONCE per manager; a second discover_and_load call replays the recorded result instead of re-walking (which would report every extension as “already loaded” — N false failures on the second host in one process).
