A handler for zxdg_output_manager_v1

This handler adds additional information to the OutputInfo struct that is available through the xdg_output interface. Because this requires binding the two handlers together when they are being created, it does not work with new_default_environment!; you must use default_environment! and create the OutputHandler outside the constructor.
