Evaluates schema against document under ISO’s #DEFAULT phase resolution: schema.default_phase if set, otherwise #ALL (every pattern) — see evaluate_with_phase and plan/05-phase-selection.md. Returns one Report per fired check, in the order patterns/rules/ checks appear in the schema and nodes are visited (document order within a rule’s matches).
