Run plugins on directives.

Executes native plugins (and document discovery) on the given directives, modifying them in-place. Plugin errors are appended to errors.

This is called by process() as part of the full pipeline, but can also be called standalone (e.g., by the LSP) when plugin execution is needed outside the normal load flow.
