Parse content into rows of fields with dialect. Port of _csv.reader’s
state machine, minus the escape character (no sniffed dialect sets one) and
strict (always off, as QUOTE_MINIMAL leaves it).
A blank line yields an empty row, as csv.reader does — upstream drops
those before building the table, where a [""] row would have drawn a
spurious blank line in it. \r never reaches here: the reader has already
applied Python’s universal newlines.
