is already ready. None unregisters; callbacks racing with removal are safe.
The host serializes execute/install/poll/cancel/unregister/destroy per handle.
After Pending, the host waits for a wake; plugins must wake for every new
opportunity to poll. Partial callbacks remain synchronous within poll only.
