Produce a summary paragraph of the transcript using a local heuristic.

This is NOT an LLM call — it uses fast pattern matching to extract:

  • Accomplishments (tool results with non-error output, assistant messages with code/data)
  • In-flight work (the most recent user message)
  • Blockers (tool results with errors)

Returns a SummarizedContext with the paragraph and message count.
