Collect all cross-table exclusive constraint trigger specs for schema.

Returns one ExclTriggerInfo per (interface exclusive constraint, concrete implementor). The same fn_name may appear multiple times (once per implementor); callers should deduplicate when emitting CREATE OR REPLACE FUNCTION.
