Resolve the effective top-level range.

The Range header supplies the base range; the limit and offset query parameters take precedence over it when present, matching PostgREST. The server-configured max_rows is then applied as a ceiling, so a request that asks for no limit – or for more rows than the server permits – cannot pull an unbounded result set into memory.
