Read surface for the Ledger (stable, versionable).
Canonical amounts are represented as integer quantities:
- IDs: fixed64/uint32
- Ledger amounts: U128 (hi/lo) at fixed 18-decimal scale
- Chart series: scaled integer vectors (e.g. *_q) with field-specific scales
REST JSON responses render human-friendly decimal strings without duplicating REST message types here.

§subaccount_id: Option<u64>Optional sub-account to scope balances to. If omitted, balances are returned for the authenticated root account derived from the JWT or API key. The logical account id itself is always derived from the caller’s JWT or API key, not from this payload.

This impl uses serialize_map(None) because the number of emitted
fields depends on default-omission rules; serializers that require
known map lengths (e.g. bincode) will return a runtime error.
Use the owned message type for those formats.
