The instance hash of a program whose canonical hash is own, under
ancestors whose canonical hashes are ancestors, innermost first:
SHA-256 over a versioned tag, own, and each ancestor’s hash in
order. PolydatProgram::instance_hash is this over interpreter
programs; a host holding kernels of any engine passes each kernel’s
Kernel::canonical_hash and gets
the same value (scope_model.md §8).
