Parse a slash command from a comment body.
Scans each line for a /command at the start, optionally prefixed by an
@orch or @orchestrator mention on the same line. Returns the first
valid command found. Unknown /something lines are skipped. Lines inside
markdown fenced code blocks (``` or ~~~) are ignored to prevent accidental
command execution from code examples. Per CommonMark, a backtick fence
closes only a backtick fence and a tilde fence closes only a tilde fence —
mismatched closers are ignored.
